How It Works:
The group insurance program is underwritten by Industrial Alliance for Life, Long Term Disability and Short Term Disability, SSQ for Accidental Death & Dismemberment and Green Shield Canada for Extended Health and Dental benefits. Employee and HR Support Solutions are provided by Morneau Shepell and is included as part of the Extended Health benefit.
The group benefits plan is designed on a building-block approach. Each employer can choose the plan which is best suited to the needs of its staff. Options are as follows:
- Core Plan includes Life insurance, Accidental Death & Dismemberment insurance and Extended Health insurance, plus Employee and HR Support Solutions and Best Doctors.
- Elective benefits include everything in the Core Plan plus the option to elect Long Term Disability and/or Dental Care benefits and/or Short Term Disability insurance.
BENEFIT SUMMARY
BASIC LIFE INSURANCE
- Pays in units of $10,000 in the event of death, to a maximum benefit of $250,000
BASIC ACCIDENTAL DEATH & DISMEMBERMENT (AD&D) INSURANCE
- Pays an additional amount equal to the Life Insurance benefit, to a maximum benefit of $250,000
- Covers both occupational and non-occupational accidents
- Pays a portion of the death benefit for dismemberment or paralysis due to an accident
LONG TERM DISABILITY INSURANCE
- Pays in units of $500 to a maximum benefit of $3,000 per month
- Benefits start on the 120th day of continuous disability and continue to age 65
- Payments offset by workers’ compensation and disability benefits under CPP or QPP
LTD Benefit Tax Status...
- Tax status of the LTD benefit is determined by whoever pays the premium.
- If the Plan Member pays 100% of the premium, the benefit is considered non-taxable since it does not attract income tax if they become disabled.
- When the Plan Sponsor pays the premium (or any portion of the premium) the benefit is taxable when collected.
- Note: Tax Status will effect LTD benefit premium pricing
EXTENDED HEALTH BENEFITS
- There is a deductible of $25 per person up to $50 per family for this benefit
- Unlimited overall maximum for all health benefits (other than out-of-country expenses and drug expenses)
- Each employee can choose single or family coverage or waive coverage if there is comparable coverage under a spouse’s plan
- Medical Services and Supplies
- 100% coverage for services of paramedical practitioners, up to $500 per practitioner per calendar year
- 100% coverage for eye glasses or contact lenses, up to $100 every 24 months
- 100% coverage for eye exams, up to $70 every 24 months
- Coverage for hearing aids, private duty nursing, medical equipment and supplies
- Drug Coverage
- 100% coverage for prescription drugs, including oral contraceptives, up to $2,000 per calendar year
- A dispensing fee deductible applies for each prescription
- Hospital Accommodation
- 100% coverage for semi-private hospital accommodation in Canada
- Out-of-Country Coverage
- 100% coverage for emergency out-of-country medical costs, up to a per incident maximum of $5,000,000 (trips with maximum 60 days duration)
- Employee & Family Assistance Program (EFAP)
- Access to qualified professionals that provide counseling and resources for support when individuals have personal family or work-related concerns
- HR Support Solutions (HRSS)
- On-demand access to HR management services
- Best Doctors
- Access to Best Doctors, a referral service for individuals who have been diagnosed with a serious illness.
DENTAL BENEFITS
- All dental benefits are limited to a combined maximum of $1,000 per covered person per calendar year
- Benefits based on the current Provincial dental association fee guide
- A deductible of $25 per person up to $50 per family applies
- Each employee can choose single or family coverage, or can waive this coverage if there is comparable coverage under a spouse’s plan
Dental Services Include:
- 100% coverage for basic dental services such as check-ups, x-rays, fillings and basic dental surgery
- 100% coverage for comprehensive basic services including endodontic & periodontic services
WEEKLY INDEMNITY INSURANCE
- Benefit amount is 66.67% of weekly earnings to a maximum benefit equal to the current EI maximum
- Benefits start on the 8th day of continuous disability and continues for up to 16 weeks
- Payments are offset by workers’ compensation, EI, and CPP or QPP
